IL Youth Survey

In the coming weeks, schools are administering the IL Youth Survey to 8th-12th graders. Here is a sample report from previous surveys. The questions reference drug use, gambling, and mental health. Contact your school for steps on how to opt-out. This form is available to fill out and submit to school administration for future opt-out on surveys, personal data mining, etc. Awake Illinois recommends it be submitted prior to the start of each school year and again at semester break.
